Cagrilintide (AM833) is a stable, lipidated, long-acting synthetic analogue of human amylin, N-terminally acylated with a γGlu linker and a C20 fatty diacid moiety, carrying helix-stabilizing substitutions that form an intramolecular E14–R17 ionic lock stabilizing its N-terminal α-helix along with three amino acid substitutions relative to pramlintide (N14E, V17R, P37Y), and reported to have a plasma half-life of approximately 180 hours.
Research Background
Cagrilintide has been characterized in a range of pre-clinical and clinical research settings. In vitro pharmacological studies employing HEK293 and COS-7 cell transfection systems and cAMP response-element (CRE) reporter assays in BHK cells compared cagrilintide's activity across 25 endpoints against reference ligands including pramlintide, salmon calcitonin, and human calcitonin. Structural investigation using cryo-electron microscopy resolved cagrilintide–receptor complexes at resolutions of approximately 2.2–3.0 Å, complemented by molecular dynamics simulations of two receptor complexes (cagrilintide-bound and rat-amylin-bound AMY3R–Gαs), each modeled in triplicate for a combined total of 6 microseconds. In vivo experiments have been conducted in rodent models, including high-fat-diet-fed wild-type mice and RAMP1/3 double-knockout mice, and the compound has also advanced to Phase 1 and Phase 2 human clinical trials as documented in the published literature.

Once a vial is reconstituted with bacteriostatic water, it should be stored in the refrigerator to help maintain stability. Under these conditions, reconstituted material is generally considered stable for up to 30 days.
Lyophilization is a dehydration technique in which compounds are frozen and then exposed to low pressure. This causes the water in the vial to sublimate directly from solid to gas, leaving behind a stable, crystalline white structure. This powder can be kept at room temperature until reconstitution.
Upon receipt, products should be stored away from heat and light. For short-term use, refrigeration at approximately 4°C (39°F) is suitable.
